Release of Information (ROI)

Would you like someone to be able to communicate with us about your services and help support your care? You can do this by submitting a Release of Information (ROI) form. An ROI allows you to give permission for certain information about your services or support plan to be shared with individuals or organizations you choose.

This form lets you specify what information may be shared, who it may be shared with, and the reason for sharing it. It helps us coordinate services effectively while continuing to protect your privacy.

Growth Unlimited requires written consent before sharing any personal or service-related information with third parties such as family members, guardians, medical professionals, case managers, or other service providers. This ensures that we remain compliant with HIPAA privacy regulations and safeguard your confidential information.

How to Submit Your ROI
